WHO MAY BE NOMINATED
The Outstanding Filipino Physicians (TOFP) award is open to every Filipino Physician (natural born or naturalized) who is at least forty one (41) years of age on the 1st day of January of the year when the award is given, of good moral character, and whose dedication to the medical profession has made significant contribution to the advancement of the medical profession, to public health, to national development anchored on the sixth tenet of the Jaycee Creed:”That Service to Humanity is the best work of life.”
CRITERIA
TOFP awardees shall be chosen on the strength of their character, integrity, commitment and dedication to the medical profession focusing on the impact of their recent and past achievements that manifest their caring and sharing for their countrymen, instill patriotism by staying in the country in the service to the community particularly to the indigents and for making significant contributions to national development in general, and the public welfare in particular.
SCREENING AND JUDGING
Nominations shall be screened by the Screening Committee that shall ascertain the technical compliance to the Nomination Book rules and guidelines and to the basic requirements for Nominees.
Judging on the other hand shall be made by a Board of Judges composed of distinguished individuals. The decision of the Board of Judges shall be final and unappealable.
NOMINEES’ COMMITMENT
Awardees shall be informed of the date and venue of the awarding ceremonies. In signing the Nomination Form, the nominees bind themselves to be present at the awarding ceremonies if chosen as awardees, barring any unforeseen events.
NOMINATION
Nominees to the TOFP award shall be pre-screened using the “Nomination Book” submitted by the NOMINATOR.
The Nominator shall cite in the Nomination Book, the nominee’s background, scope of medical activities and exceptional achievements. Honors, citations and other awards received by the nominee may also be included as attachments to the Nomination Book subject to certain limitations below enumerated. (Please refer to Criteria)
The nominator must highlight how the nominee’s achievements in the practice of medicine have benefited mankind and how it conforms with the 5th and 6th tenets of the Jaycee Creed, viz: “That earth’s great treasure lies in human personality;” and, “That service to humanity is the best work of life.”

PERSONALITY
1. Professional ethical conduct and practices
2. Leadership in the community
3. High personal integrity
4. Self service to those in need without regard to pecuniary considerations
5. Awards and citations received related to the medical profession.
SERVICE TO HUMANITY
1. Contribute greatly to public health and community Medical services that impact on the Philippine Millennium Development Goal targets, either individually, jointly with others or in cooperation with Governmental and non-governmental organizations.
such as:
a. Eradicate extreme poverty and hunger
b. Achieve universal primary education
c. Promote gender equality and empower women
d. Reduce child mortality
e. Improve maternal health
f. Combat HIV AIDS, malaria and other diseases
g. Ensure environmental sustainability
h. Develop a Global Partnership for Development
2. Exemplary achievements/practices that brings about
better health outcomes, more responsive health
systems and equity in health care financing.
3. Special or related projects that relates to acts of heroism in the health of communities, special or marginalized populations, during calamities, disasters and/or outbreaks beyond the physician’s call of duty.
4. Outstanding leader in his field, has high regard for Personal integrity and respect for human rights in the practice of medicine
5. Contributions to Human Resources for Health Policy Agenda and/or advocacy efforts against “brain drain” in the medical professions by choosing to continuously serve and care for our countrymen notwithstanding the lure of greener pasture overseas.
6. Research and development works in areas of health aimed at promoting better health outcomes and moreresponsive local health systems.
7. Journals or published articles acknowledged/presented in a local and/or international fora; or acknowledged by a professional organization, the academe, or the Department of Health as an original work.
NOMINATION BOOK REQUIREMENTS:
1. Must be uniform in size and must not exceed 8 ½” x 14,” fastened or bounded, with nominee’s picture pasted and name printed on the cover;
2. Must not exceed forty one (41) pages including the executive summary, printed or typewritten using a minimum font size of 12 points;
3. Must not have more than seven (7) multi pages (a single document or letter attached consisting of more than one page shall not be considered a multiple page for purposes of this rule). Each multiple page must not exceed eighteen (18) pages.
4. Must be submitted original with six (6) additional reproduction or photocopies (the photo pasted on the front page of each copy must be original and taken not more than six (6) months from submission of the Nomination Book.
5. Must be accompanied by a duly accomplished Nomination Form bearing the signatures of the Nominator and the Nominee.
SUBMISSION OF ENTRY
Accomplished nomination form and all supporting documents must be submitted on or before
